CoreLink ONE is in Phase 1 of development. Currently, only the Community Health Worker (CHW) education and training modules are live and functional. All other features—including community member profiles, mobile applications, case management CRM, housing coordination, and AI navigation tools—are planned roadmap items and are currently inactive for all users until further system validation is complete.
PHI processing is strictly disabled in this phase. Functions for care plans, referrals, appointments, and secure messaging will activate only after comprehensive HIPAA, legal, and technical validation to ensure platform safety and data integrity.
Patient App
The Patient Hub mobile app (Planned/Inactive) will offer community members transparency in their care journeys. Features like housing status and secure records are roadmap targets. Currently, only CHW education is active; PHI processing is disabled pending HIPAA and technical validation.
